WHAT IS A TRAVEL DOCUMENT ISSUED BY THE EMBASSY?
- It is a paper document(not a passport) issued by the Embassy to Cambodian Nationals.
- It is a single-use travel document to enter Cambodia.
- It is only valid for three(3) months from the date of issued.
WHO CAN APPLY? Cambodian traveling back to Cambodia with the following situation below.
- Cambodian with expired/nearly expire Cambodian Passport *must have a valid US green card/residency card.
- Cambodian lost the Cambodian Passport *required to submit a police incident report.
WHEN TO APPLY?
- one(1) month before the intended date of entry to Cambodia.

- 2-3 weeks

REQUIREMENTS
- Completely filled-up and signed Temporary Travel Document form (one copy)
- Photocopy of Cambodian passport bio-page *passport name & photo information
- 2x2 Passport photo (2 pieces)
- Photocopy of valid US residency card/green card.
- Travel itinerary (Optional)
- Police incident report (original copy) in case of a lost passport only*
- Prepaid return envelope with official receipt (for mailing applicant only)
