Temporary Travel Document/Laissez Passer (For Cambodians Only) 

​Certified certificate for children born abroad (For CAmbodians)

This request is for a child who is born abroad to Cambodian parent(s) and who needs to obtain a Cambodian birth certificate in Cambodia. 

Required documents : 

• Completely filled-out and signed certified birth certificate request form (one copy)

• One 2x2 passport photo of the child
• Bio-page copy of parent's passport 
• Copy of parent's valid Green Card (if applicable) 
• Fee of 5$ (Cashier's check or money order)

​Transportation of cremated remains

• Required documents to apply for the Embassy's letter:

1. Request letter with contact information from the decedent's family including burial location in Cambodia
2. Certified copy of death certificate  *Do not send original copy
3. Copy of Cambodian passport or identification card of the decedent that contain the name in Cambodian 
4. Letter certifying the cremation from a funeral home
5. Fee ​in cashier's check or money order payable to the Embassy of Cambodia

​TRANSPORTATION OF Humain ​REMAINS

Required documents to apply for the Embassy's letter:

1. Request letter with contact information from the decedent's family including burial location in Cambodia
2. Copy of Cambodian passport or identification card of the decedent that contain the name in Cambodian 
3. Certified copy of death certificate  *Do not send original copy
4. Burial/Transit Permit 
5. Embalmers Affidavit
6. Letter of non-communicable disease
7. Air ticket 
8. Fee ​in cashier's check or money order payable to the Embassy of Cambodia

TRANSLATION OF BIRTH CERTIFICATE (for Cambodians)

• Request letter
• Original birth certificate
• Fee $50 USD

• Authenticated Department of State's Letter (original copy) and attached one copy of whole document
• Fee
  - Money order or certified cashier's check only - payable to Embassy of Cambodia
