TRANSPORTATION OF CREMATED REMAINS
Required documents to apply for the Embassy's letter:
- Request letter with contact information from the decedent's family including burial location in Cambodia
- Original or certified death certificate
- Copy of Cambodian passport or identification card of the decedent that contain the name in Cambodian
- Letter certifying the cremation from a funeral home
- Fee in cashier's check or money order payable to the Embassy of Cambodia
TRANSPORTATION OF HUMAN REMAINS
Required documents to apply for the Embassy's letter:
- Request letter with contact information from the decedent's family including burial location in Cambodia
- Copy of Cambodian passport or identification card of the decedent that contain the name in Cambodian
- Original or certified death certificate
- Burial/Transit Permit
- Embalmers Affidavit
- Letter of non-communicable disease
- Air ticket
- Fee in cashier's check or money order payable to the Embassy of Cambodia
TRANSLATION OF CAMBODIAN BIRTH CERTIFICATE
- Request letter
- Original birth certificate
- Fee $50 USD
DOCUMENT AUTHENTICATION
- Authenticated Department of State's Letter (original copy) and attached one copy of whole document
